Fetal Liver Masses
· Hepatic cyst- Hepaticcysts are not uncommon.
· Choledochal cyst- These cysts vary in size and location within the liver or juxtaposition to the liver. The cyst appears in the expected location of the bile ducts. Tubular structures are seen entering the cyst.
· Mesenchymal hepatic hamartoma- Usually present as solitary round lesions with well defined margins consisting of multiple cysts of variable sizes, separated by thick or thin septa. Antenatal detection has been reported at 25 weeks.
· Hemangioma-These may be solid, cystic or complex. Cavernous hemangiomas are the most common: high output failure, hydrops and intrauterine death have been reported. Doppler US may demonstrate increased flow within these masses, and help differentiate cystic tumors from other cystic masses.
· Hepatic teratoma- In common with teratomas elsewhere these may vary from solid to completely cystic masses, although most tumors are complex.
· Portal vein aneurysm- This is usually seen as a focal dilatation of the portal vein; color flow Doppler/pulsed Doppler sonography demonstrates active blood flow within the mass.
· Hepatoblastoma- Usually a solid tumor.
· Hepatic adenoma- Very rare in the newborn but have been reported in utero. Adenomas are seen as hypoechoic masses with irregular margins. Adenomas reported in utero may be associated with maternal treatment with steroids and other hormones during pregnancy.
